Pass to Play: Annual Sports Physical Event for Youth Athletes
Are you ready for the season?
Golden State Orthopedics & Spine’ annual event provides pre-participation evaluation and optional baseline concussion screenings to area youth.

All attendees must also download and complete the Pre-Participation Physical Evaluation form, and bring it with them to the event. A copy of the exam will be sent to each student athlete’s personal physician.
Optional cardiac screenings will be offered.
*All proceeds from this event are donated back to the students’ school athletic programs. To date, GSOS has donated over $26,000 to local athletic programs.
